This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Understanding Your Health Information

Each time you visit a healthcare provider, a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment, and a plan for future care or treatment. This information, often referred to as your health or medical record, serves as a:

  • Means of communication among the many health professionals who contribute to your care
  • Legal document describing the care you received
  • Tool in educating health professionals
  • Source of data for medical research
  • Basis for public health officials who work to improve the health of the nation

You have the right to request an accounting of certain disclosures of your health information.

Right to Request Restrictions

You have the right to request a restriction or limitation on the health information we use or disclose about you.

Right to Request Confidential Communications

You have the right to request that we communicate with you about health matters in a certain way or at a certain location.

You have the right to a copy of this notice. You may ask us to give you a copy of this notice at any time.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. To file a complaint with us, contact our Privacy Officer.

Important Note

You will not be penalized for filing a complaint.

Changes to This Notice

We reserve the right to change this notice. We reserve the right to make the revised or changed notice effective for health information we already have about you as well as any information we receive in the future.
